One underseen (but nonetheless impactful) villain of the X-Men may be the best evil twin in Marvel history. The evil twin trope is one that is commonly seen and used throughout storytelling, particularly in the Marvel Universe. The likes of Iron Man and Deadpool have dealt with evil twins of themselves, while the similarly themed evil doppelgänger trope is a common one in superhero culture as a whole.
However, if one villainous evil twin can be named as the most dastardly of them all, it's Cassandra Nova, sister to X-Men's original leader, Professor Charles Xavier. On top of having a backstory that changed Marvel lore significantly, Cassandra Nova's brutal approach was enough to shake the X-Men to their core.
Cassandra Nova not only orchestrated an entire genocide on the island of Genosha, she also forced the normally peaceful Xavier to consider extreme violence as a last resort against his own sister. Cassandra Nova Xavier's first appearance comes in New X-Men #114 by Grant Morrison, Frank Quitely, Tim Townsend, Brian Haberlin, Comicraft, Pete Franco, and Mark Powers. She immediately makes an impact in her villain debut by campaigning for a mutant genocide in her first appearance, then committing that genocide in the very next issue. Acquiring the services of Donald Trask III, who controls the Wild Sentinels, she expounds the need to "save the human species" from mutantkind. In the next issue, a Sentinel attack on Genosha resulted in 16 million casualties.
Nova is described best by the Shi'ar as a Mummudrai, the manifestation of a powerful mutant that is both equal and opposite to that mutant. The power potential of Xavier made it possible for him to produce his Mummudrai as a physical form while still in the womb, where his first battle commences. Conceived as an evil entity, Cassandra attempts to strangle Charles with the umbilical cord. To save himself, he produces a psionic blast big enough to fling their mother down the stairs, essentially killing his twin. Despite being stillborn, Cassandra's regeneration abilities allow her to spend decades rebuilding her body.
It's worth noting that, in addition to having a healing factor that is matched by Wolverine, Cassandra Nova has a grasp on telepathy that is equal to that of Charles Xavier. Being his Mummudrai makes Cassandra the perfect carbon copy of everything Xavier is capable of as a mutant, including telepathy. As such, Nova suffers from the same weaknesses that Xavier is capable of, as well. For example, she's incapable of using her psychic powers on anyone wearing Magneto's helmet, as seen in X-Men: Red #10 by Tom Taylor and Rogê Antônio.

What makes Professor X such an excellent leader is his optimism for the future and idealistic vision - his dream - for mutantkind to be treated equally to humanity. He can bring people to believe in his dream because he believes so strongly in it through conviction. His twin sister has those same skills as an orator, but her vision of the future rooted in self-hate and venom for mutantkind makes hers far more terrifying.
The same way that Xavier can speak peace into power, Nova can speak destruction into power with a message of hate. The fact she is able to do just that in her first pair of appearances is proof of that. Her first panel depicts her giving a hateful speech that comes to fruition in the next issue. Xavier preaches a peaceful philosophy and is able to produce results, making him an effective leader. Nova, alternatively, preaches hate and produces genocide instantaneously. She's simultaneously effective and scary in her first appearances.

Beyond just committing a game-changing genocide that continues to be referenced in today's X-Men stories, the impact that Cassandra Nova has had on other characters cannot be denied, especially regarding her brother. Charles Xavier has always been adamant about not only preaching peace, but preserving life, even passing a law on Krakoa that forbids killing humans. It is rare to see Xavier deviate from this rule, and if he does, that's when readers recognize just how high the stakes are. He's broken this rule for Cassandra Nova not once, but twice.
It was already mentioned earlier that the X-Men leader breaks his biggest rule before he is even born in an effort to stop his twin sister's evil from spreading out into the world. His attempt fails, but he gets another chance in New X-Men #116, where he feels compelled to fire six bullets into his sister. It's uncharacteristic of Xavier to murder someone in cold blood like this, but it speaks to exactly how dangerous he's deemed his twin to be only a couple of issues into her debut.
